CCMGA ENTRY FORM
TWO MAN TEAM CHAMPIONSHIP

Saturday - Sunday, November 18-19, 2006

FTee times are the same on both days, but your Tee Box changes.
if you start on # 1 on Saturday, you will start on # 10 on 
Sunday, and vice versa.

FORMAT: THIRTY-SIX HOLES – GROSS & NET – TEN STROKE MAXIMUM HANDICAP 
DIFFERENTIAL - BLUE TEES ONLY – OPTIONAL GROSS AND NET SKINS – 
CTP'S ON ALL PAR THREES. 

SATURDAY: FRONT NINE (HOLES 1 thru 9), SCRAMBLE 
25% OF COMBINED TEAM HANDICAP. 

BACK NINE (HOLES 10 thru 18), CHAPMAN/PINEHURST 
(HANDICAPS: 60% OF THE LOW H/CAP AND 40% OF THE HIGH H/CAP). 

SUNDAY: FRONT NINE, (HOLES 1 thru 9), BEST BALL
FULL HANDICAPS

BACK NINE, AGGREGATE (COMBINED SCORE) 
FULL HANDICAPS

SCRAMBLE FORMAT	

Each player tees off, they then decide upon which 
ball to use. They then both play their next shot from that location, 
and it continues in this manner until the ball is holed.

The ball may be placed within 1 club length of the chosen location, 
and the ball must stay in the same conditions  . .i.e., if the chosen 
ball is in the rough, it must stay in the rough, if in a bunker, 
it must stay in the bunker, etc. On the green, the ball must stay 
within 1 putter head distance from the chosen ball. (This is the only 
format in this tournament where the ball may be picked up and placed)

CHAPMAN / PINEHURST		

Each player tees off, and then they 
each go to the other players ball and hit it. After they have both 
hit their second shots, they decide which ball location to use for 
an alternating shot format until the ball is holed. If player ‘A’s 
ball is chosen, then it must be hit by player ‘B’, and vice versa.
Ball must be played as it lies.

BEST BALL		

Each player plays his own ball all the way 
into the hole, and then the lower of the 2 scores is used as the 
team score. The Sunday scores will be posted as tournament scores.

AGGREGATE 	

Each player plays his own ball all the way into 
the hole, and then the 2 scores combined are used as the team score. 
The Sunday scores will be posted as tournament scores.
